The nose is an area of the body that contains many tiny blood vessels (or arterioles) that can break easily. In the United States, one of every seven people will develop a nosebleed some time in their lifetime. Nosebleeds can occur at any age but are most common in children aged 2-10 years and adults aged 50-80 years. Nosebleeds are divided into two types, depending on whether the bleeding is coming from the front or back of the nose.

What is an Anterior Nosebleed?

Most nosebleeds (or epistaxes) begin in the lower part of the septum, the semi-rigid wall that separates the two nostrils of the nose. The septum contains blood vessels that can be broken by a blow to the nose or the edge of a sharp fingernail. Nosebleeds coming from the front of the nose,  (anterior nosebleeds) often begin with a flow of blood out one nostril when the patient is sitting or standing.

Anterior nosebleeds are common in dry climates or during the winter months when dry, heated indoor air dehydrates the nasal membranes. Dryness may result in crusting, cracking, and bleeding. This can be prevented by placing a light coating of petroleum jelly or an antibiotic ointment on the end of a fingertip and then rubbing it inside the nose, especially on the middle portion of the nose (the septum).

How Do I Stop an Anterior Nosebleed?

  • Stay calm, or help a young child stay calm – a person who is agitated may bleed more profusely than someone who’s been reassured and supported
  • Keep head higher than the level of the heart – sit up
  • Lean slightly forward so the blood won’t drain in the back of the throat
  • Gently blow any clotted blood out of the nose – spray a nasal decongestant in the nose
  • Using the thumb and index finger, pinch all the soft parts of the nose.  Do not pack the inside of the nose with gauze or cotton
  • Hold the position for five minutes – if it’s still bleeding, hold it again for an additional 10 minutes

What is a Posterior Nosebleed?

More rarely, a nosebleed can begin high and deep within the nose and flow down the back of the mouth and throat, even if the patient is sitting or standing.

Obviously, when lying down, even anterior (front of nasal cavity) nosebleeds may seem to flow toward the back of the throat, especially if coughing or blowing the nose. It is important to try to make the distinction between the anterior and posterior nosebleed, since posterior nosebleeds are often more severe and almost always require a physician’s care. Posterior nosebleeds are more likely to occur in older people, persons with high blood pressure, and in cases of injury to the nose or face.

What are the Causes of Recurring Nosebleeds?

  • Allergies, infections, or dryness that cause itching and lead to picking of the nose
  • Vigorous nose-blowing that ruptures superficial blood vessels
  • Clotting disorders that run in families or are due to medications
  • Drugs (such as anticoagulants or anti-inflammatories)
  • Fractures of the nose or the base of the skull – head injuries that cause nosebleeds should be regarded seriously
  • Hereditary hemorrhagic telangiectasia, a disorder involving a blood vessel growth similar to a birthmark in the back of the nose
  • Tumors, both malignant and non-malignant, have to be considered, particularly in the older patient or in smokers

When Should an Otolaryngologist be Consulted?

If frequent nosebleeds are a problem, it is important to consult an otolaryngologist. An ear, nose, and throat specialist will carefully examine the nose using an endoscope, a tube with a light for seeing inside the nose, prior to making a treatment recommendation. Two of the most common treatments are cautery and packing the nose. Cautery is a technique in which the blood vessel is burned with an electric current, silver nitrate, or a laser. Sometimes, a doctor may just pack the nose with a special gauze or an inflatable latex balloon to put pressure on the blood vessel.

Tips to Prevent a Nosebleed:

  • Keep the lining of the nose moist by gently applying a light coating of petroleum jelly or an antibiotic ointment with a cotton swab three times daily, including at bedtime. Commonly used products include Bacitracin®, A and D Ointment, Eucerin®, Polysporin® and Vaseline®
  • Keep children’s fingernails short to discourage nose-picking
  • Counteract the effects of dry air by using a humidifier
  • Use a saline nasal spray to moisten dry nasal membranes
  • Quit smoking – smoking dries out the nose and irritates it

Tips to Prevent Rebleeding after Initial Bleeding has Stopped:

  • Do not pick or blow nose
  • Do not strain or bend down to lift anything heavy
  • Keep head higher than the heart

If Rebleeding Occurs:

  • Attempt to clear nose of all blood clots
  • Spray nose four times in the bleeding nostril(s) with a decongestant spray
  • Repeat the steps to stop an anterior nosebleed
  • Call a doctor if bleeding persists after 30 minutes or if nosebleed occurs after an injury to the head

Nonallergic rhinitis occurs when blood vessels in your nose expand (dilate), filling the nasal lining with blood and fluid. There are several possible causes of this abnormal expansion of the blood vessels or inflammation in the nose. But, whatever the trigger, the result is the same — swollen nasal membranes and congestion.

Many things can trigger the nasal swelling in nonallergic rhinitis — some resulting in short-lived symptoms while others cause chronic problems. Nonallergic rhinitis triggers include:

Environmental or occupational irritants. Dust, smog, secondhand smoke or strong odors, such as perfumes, can trigger nonallergic rhinitis. Chemical fumes, such as those you might be exposed to in certain occupations, also may be to blame.

Weather changes. Temperature or humidity changes can trigger the membranes inside your nose to swell and cause a runny or stuffy nose.

Infections. A common cause of nonallergic rhinitis is a viral infection — a cold or the flu, for example. This type of nonallergic rhinitis usually clears up after a few weeks, but can cause lingering mucus in the throat (postnasal drip). Sometimes, this type of rhinitis can become chronic, causing ongoing discolored nasal discharge, facial pain and pressure (sinusitis).

Foods and beverages. Nonallergic rhinitis may occur when you eat, especially when eating hot or spicy foods. Drinking alcoholic beverages also may cause the membranes inside your nose to swell, leading to nasal congestion.

Certain medications. Some medications can cause nonallergic rhinitis. These include aspirin, ibuprofen (Advil, Motrin IB, others), and high blood pressure (hypertension) medications, such as beta blockers. Nonallergic rhinitis can also be triggered in some people by sedatives, antidepressants, oral contraceptives or drugs used to treat erectile dysfunction. Overuse of decongestant nasal sprays can cause a type of nonallergic rhinitis called rhinitis medicamentosa.

Hormone changes. Changes in hormones due to pregnancy, menstruation, oral contraceptive use or other hormonal condition such as hypothyroidism can cause nonallergic rhinitis.

Stress. Emotional or physical stress can trigger nonallergic rhinitis in some people.

Nasal obstruction is a symptom rather than a standalone diagnosis, and there is a wide range of medical and structural problems that could cause such a symptom. Medical conditions include bacteria, infection, or illness. Structural conditions could indicate a nasal septal deformity, nasal polyps, or a problem with the turbinate bones.

A deviated septum is a common condition that involves a displacement of the septum, the wall that separates the nostrils, to one side of the nose. About 80 percent of people have a deviated septum, which often develops as a result of an injury to the nose. This condition makes one nasal passage smaller than the other, which can affect breathing if the displacement is great enough.


Patients with a severe deviated septum may experience nasal congestion, nosebleeds and frequent or recurring sinus infections as a result of their uneven nasal passages. Those with only minor displacement may not even be aware that they have a deviated septum and experience no symptoms.

Treatment for a deviated septum can vary depending on the severity of the condition and the symptoms associated. For most patients, this condition can be managed through decongestants and antihistamines that aim to reduce nasal congestion. For more severe cases, an ear nose and throat physician may recommend surgery to correct the displacement. Surgery involves a procedure called a septoplasty to reposition the septum in the center of the nose. This procedure is often performed in conjunction with rhinoplasty, or nose reshaping surgery.

Nasal polyps are soft, painless, noncancerous growths on the lining of your nasal passages or sinuses. They hang down like teardrops or grapes. They result from chronic inflammation due to asthma, recurring infection, allergies, drug sensitivity or certain immune disorders.

Small nasal polyps may not cause symptoms. Larger growths or groups of nasal polyps can block your nasal passages or lead to breathing problems, a lost sense of smell, and frequent infections.

Nasal polyps can affect anyone, but they're more common in adults. Medications can often shrink or eliminate nasal polyps, but surgery is sometimes needed to remove them. Even after successful treatment, nasal polyps often return.

Dr. Hirsch is awesome! My husband and my kids are now 19 and 15, and Dr. Hirsch put ear tubes in their ears when they were both babies. When our daughter was 4, he removed her tonsils and adenoids. It has been 14 years since I had my thyroidectomy at Riverside with Dr. Michael Martyn. He had the best bedside manner. He took extra time to answer all my questions. My scar is not noticeable. During my first surgery he removed my left thyroid that had a large goiter. He was conservative due to my age being 25 at the time. Only to find out my pathological results came back that I had thyroid cancer. He called me to inform me of the results and took me back to surgery.He removed my thyroid lobe on the right side and some lymphnodes that came back clear. I was very lucky that my Endocrinologist Dr. Angela Bucci referred me to him. Since the surgery I became a RN myself and I work in surgery in my hometown. My family has seen doctors at this facility for almost 10 years. My youngest son has had 3 procedures (tubes/tonsils) by Dr. Jeffrey Hall; I've seen Dr. Hall for my own sinus/allergy issues as well. I have have a full panel screening done for allergies as well. This past month I had a full thyroidectomy due to large nodules on the thyroid. I was referred to Dr. Alfred Fleming and could not have been more pleased. I never felt rushed, asked all questions, everything was thorough and resources provided. His medical staff was incredibly helpful. I had some reactions due to low calcium a few days later over the Easter holiday weekend and called the on-call doctor. Dr. Willett was wonderful.
